Tensei Isekai Mad Libs

You may notice that many isekai plots are rather formulaic, especially the "tensei isekai" variant that involves protagonists being reincarnated in another world. I decided to let you try your hand at making your own.

Here are the questions to fill out. You can make the answers relatively ordinary, or make them as silly as possible. Like most Mad Libs, it's most fun when you're reading them to one or more other people (who don't know how their answers will be used), who offer their own answers.
  1. Give your character a name, preferably a Japanese one, and decide the character's gender- male, female or nonbinary. The character will henceforth be referred to as "Protagonist," or the pronoun "they" and its variants, which can be altered as appropriate. I know Mad Libs generally don't explain where each answer comes in, but this is a special case.
  2. A number.
  3. A job; "student" or "unemployed" counts.
  4. A cause of death, the weirder the better.
  5. A god or other higher power.
  6. A piece of modern technology.
  7. Give another name to your protagonist, who will hereafter be called Hero. You can also choose another gender if you want, and it can be different from their first life, but the protagonist will still be called "they"(again, replaced with whatever pronoun you chose).
  8. A social caste from a medieval fantasy world- peasant, merchant, knight, noble, royalty, etc.
  9. An occupation.
  10. A name for a world.
  11. The title of a work of fiction.
  12. A fictional medium- a book, a video game, a visual novel, etc.
  13. A role in the story- protagonist, antagonist, love interest, sidekick, comic relief, etc.
  14. An action that one undertakes while on a quest- bring back, kill, destroy, rescue, etc.
  15. An objective of said quest- a treasure, a princess, a dragon, etc.
  16. The goal that a quest hopes to result in- e.g. ending a war, saving the word, etc.
  17. A famous work of fiction
  18. Another number, which can be different from the first but doesn't have to be.
  19. One last number.
After submitting all your answers, plug them into the respective slots, and enjoy the result!

(Protagonist), a (Number 1)-year old (Job), suddenly died one day from (Cause of Death). (Higher Power) took pity on (Protagonist), offering them a chance to begin a new life in another world with (Modern Technology) as an extra bonus to compensate for their untimely demise.

(Protagonist) was reborn as Hero, the child of (social caste) (occupation)s, in the world of (World). As it so happens, World is the setting of (title), a popular (medium) that (Protagonist) played in their previous life, and (Hero) is the story's (role).

As it so happened, (Hero) also inherited a mission. (Hero) must (action) (objective) in order to (Goal), a ripoff of (Fantasy Work Of Fiction). This will likely be a very long journey, lasting (Number 2) volumes and being adapted into a (Number 3)-season anime, assuming the readers don't get tired first.
